Innovative approach to upright anterior tooth

  Abstract 


Missing mandibular anterior tooth is one of the most commonly missing teeth in the arch. The replacement of the missing tooth can be done by various techniques. Removable partial denture, fixed partial prosthesis, or prosthetic implants is widely preferred. Angulations of the teeth adjacent to the edentulous area are critical because of various factors. In cases of the FPD abutment teeth is supporting tooth. Upright tooth is preferred for better prognosis. In implant placement, sufficient bone must be there to place implant. Uprigation of the adjacent teeth is required for better implant angulation. Here, we have used innovative approach to upright mandibular anterior tooth in minimum time possible.

  Introduction Top


Hypodontia is one of the most common dental anomalies in permanent dentition.[1] In prosthetic implant placement, sufficient bone must be there to place implant. In few cases, uprigation of the adjacent teeth is required for better implant angulations.[2] In the present case, the patient reported with the complaint of space in the lower front teeth region. On clinical intraoral examination, the 31, 41, 35, 45 were missing and 71, 75, 85 were retained deciduous teeth [Figure 1]. Treatment plan was to extract 71, upright 32, 42 for creating space, and implant placement in the region of 31. Two anterior crowns on single prosthetic implant were planned.

  Preprosthetic Orthodontics Top


An innovative method for uprighting the root of lower incisors (32, 42) was done in this case. 0.012 NiTi wire was ligated to the posterior teeth; as the wire approached 32, 42, wire was passed below the distogingival wing and passed vertically between the mesial and distal wings and coming out of the bracket above the mesio-occlusal wing. The same was followed on the opposite side tooth in the reversed manner. Here, the wire was passed occlusally to gingivally. 32, 42 were ligated to the posterior tooth with ligature wire (0.010) to prevent mesial crown tipping [Figure 2]. The couple force generated in single-rooted teeth with push the crown mesially and root distally. Although crowns were lighted to the posterior segment, only root movement will occur to the distal side which is favorable. The technique is useful in case where single-rooted tooth uprighting is required. Advantage of this technique is that to wait for uprighting the tooth till reaches to the higher wire is not required. Postoperative OPG shows sufficient uprigation of the mandibular lateral incisor's root with placed prosthetic implant [Figure 3].
